2021-22 Varsity Student-Athlete Medical Forms

Please carefully read below and select the appropriate year you have been on the team. If you took a year off (opted out of athletics in 2020-21), you will need to complete everything under '3rd Year.' Underneath each year is all of the requirements that need to be completed before you will be medically cleared to participate in a varsity sport at Stevens. Additional links can be found at the bottom of the page.

All information must be completed and received by August 1 for fall sports and August 15 for winter and spring sports. Failure to comply could delay medical clearance to practice or compete in varsity intercollegiate competition at Stevens Institute of Technology. 

Every student-athlete's paperwork is manually checked by the Sports Medicine Staff and can take up to two weeks to process.

First-year student-athletes (including first-year students, first year on a varsity team or an incoming transfer):
