2022-2023 Varsity Student-Athlete Medical Forms


The varsity medical requirements for the 2022-2023 academic year will be updated by June 13, 2022. We are currently switching from ATS to Point and Click (Healthy Stevens).

1st, 3rd and 6th year student-athletes that require a physical can have their physical completed on this form and can upload that form to the new system when you gain access:
